Millions of people are taking Ozempic and Wegovy for rapid weight loss. Emerging research suggests these GLP-1 medications may also affect bone density and increase the risk of gout. This raises an important question: are we addressing the underlying causes of weight gain, or relying on a pharmaceutical shortcut?
GLP-1 medications, including Ozempic and Wegovy, were associated with a modestly higher risk of osteoporosis and gout in research presented at the American Academy of Orthopaedic Surgeons annual meeting. According to a report by NBC News, investigators reviewed five years of medical records from more than 146,000 adults diagnosed with both obesity and Type 2 diabetes.
Approximately 4% of GLP-1 users developed osteoporosis compared with just over 3% of nonusers, representing roughly a 30% relative increase in risk. Osteomalacia, a condition involving softening of the bones, occurred about twice as often among those taking GLP-1 medications. Gout rates were also higher, affecting 7.4% of GLP-1 users versus 6.6% of nonusers, about a 12% relative increase.
Dr. John Horneff, associate professor of orthopedic surgery at the University of Pennsylvania and lead author of the study, said the investigation began after some patients experienced significant tendon tears following relatively minor injuries. “People are taking these medications, and obviously there’s a tremendous amount of upside,” Horneff noted. “But with that, they start to decrease their intake of food and nutrients.”
Ozempic Side Effects: What the New Research Actually Found
One theory researchers offered is that because GLP-1 medications suppress appetite, some patients may not consume enough key nutrients such as vitamin D and calcium, which are critical for maintaining bone health. Another possibility is that rapid weight loss itself changes how the body builds and breaks down bone. Horneff compared it to what happens to astronauts in space, where the absence of gravitational load causes the skeleton to lose density because it no longer carries its usual weight.
For gout, rapid weight loss can lead to a temporary spike in uric acid, increasing the risk of gout attacks. Additionally, researchers noted that the greatest relative risk increase was observed for osteomalacia, with GLP-1 users experiencing the condition at more than twice the rate of nonusers. Consequently, the skeletal system, which most people never think about until something breaks, is emerging as an unexpected casualty of the weight loss drug revolution.
The research cannot yet prove these drugs directly cause these conditions. However, the pattern is consistent enough that the FDA already notes on the label for semaglutide that it might increase the risk of bone fractures in older adults and women. Therefore the question is no longer whether these risks exist. The question is whether the people taking these drugs are being told about them clearly enough to make an informed choice.
